get a piece of fabric - pull on it and try to tear it- some fabrics tear more easily- just as the genetics, health and integrity of each woman's perineum is different. but if you take a tough piece of fabric you can make a small cut in it- now pull on it- it tears easily- that is what happens with an episiotomy- it usually starts out as a second degree and then tears into much more.

I am glad I am not the only one who uses this as an analogy when my clients are asking about episiotomies. It takes a concept and makes it something visual for them to understand. I often see light bulbs going off in my classes with this explanation!

No one is mentioning the difference in perineums either, some have very thick muscles that do not give as readily as well as some perineum are long with lots of space and some are very short. There are too many variables. A side note where I have worked, I have not seen an OB just sit there a let a head "pop" out.

The episiotomy is better for the care provider because it is easier to stitch a straight line. Stitching a tear takes skill. I'd prefer to tear myself, and I did, but it was no biggie.

I had a personal experience with an episiotomy where I was cut because I was "going to tear." The health care provider was at a Kaiser and he didn't know me from Adam but was sure in a rush to deliver my daughter. She came out with her arm by her face and I watched in the mirror with horror as my episiotomy tore even further and created a 3rd degree tear all the way to my bottom. It took 40 stitches to sew me shut in layers. The next day every single time I would get a movement from gas in my bowel my stitches would hurt. It was horrendous!!! As I healed I swelled up so much that the stitches came apart in the middle of the incision site and left a gaping hole. The advice nurse assured me that it would heal fine regardless and I was never seen for it. Luckily it did heal but the process took far more than 6 weeks.

As a doula now I cringe at the way I was treated and wish I had known then what I know now. My second baby was born at 8lbs, 14 oz and I had a very small superficial tear. It was a very easy recovery. With my third there was no tear at all and I felt great.

The point in all of this is that there can be complications from an episiotomy that last for weeks on end or even a lifetime if the tissue doesn't heal well. I believe episiotomies are for emergencies only. I would never want another woman to go through what I went through. Episiotomies guarantee that muscle and tissue are cut whereas a tear may not go through the muscle and be very superficial and require little to no attention. Birthing in an upright position helps reduce the amount of pressure on your perineum.

While pushing, instead of pushing the head out full force give a gentle push then blow your air out as the baby's head is crowning. Do this over and over, stopping if you feel the burning is too much. My midwife taught me this method and it really works well. The baby's head eases out slowly and your perineum is able to stretch slowly, reducing tearing.

i didnt tear, or have an episiotomy, but i got stitches!! i guess it was their "gift" to my hubby. ha ha. but i would rather risk tearing. if you get an epideral, you will never know, and they fix it the same either way. if not, your body is already focused. you will not notice the tear.

Recommendations now state (what we nurse-midwives have known for a long time) that it is best to avoid an episiotomy because it increases the risk of extensive tearing.
