What happens if the baby is not breathing at first?

It is not uncommon that the baby doesn’t breathe effectively or at all from birth. Doctors and nurses use several tricks to get the baby to breathe. They suction out the baby’s nose and mouth which can stimulate a reflex and allow it to breathe. It also clears secretions from the airway. The nurses can flick the baby’s foot with their finger, allowing it to be startled and breathe. Rubbing the baby’s back with your hand will also stimulate breathing.

If none of these work, then the doctor must use artificial respirations. The doctor uses a tiny mask and a large bag that pushes air into the baby, allowing it to get oxygen and to stimulate breathing. This generally goes on for less than a minute before the baby begins to have more muscle tone and begins to breathe on its own. In severe cases, an endotracheal tube is dropped into the trachea and breathing for the baby lasts as long as is necessary to get the baby breathing.

In extremely rare cases, the baby is put on a ventilator until it can breathe on its own. Again, this is very rare and usually the first measures mentioned work to get the baby breathing.

One important thing to remember, if the baby is still attached to the umbilical cord - they often don't take that first breath, for a few minutes. They are still receiving oxygen and blood that has been supplied all along in the womb.

Often times parents will ask to postpone clamping and cutting of the cord, when this is respected... the cord usually stops pulsating within 3-5 minutes as the whartons jelly fills in and the exchange is stopped - then baby will often take a nice deep breath and then the CRY.

During this time a nurse may be cleaning the vernex off the baby, checking apgars, etc... but often times the medical team needs a gentle reminder that baby may not cry while still attached and parents need that reassurance as well.

Imagine a home water birth, baby slides from water to water and is gently brought to the surface then takes that breath!

Excellent post doularena and we had no idea that some parents ask to postpone clamping. I always though the baby needed to breathe immediately upon exit. Of course, my wife knew this, but you'd think after being immersed in babies and pregnancies over the last 15 months that I would have picked this up!

The baby can still breathe with the cord not clamped. It's just not an absolute requirement b/c baby is still getting blood and oxygen from the placenta. So not clamping the cord gives a little leeway if baby needs help transitioning! Baby also gets its total blood volume if you wait to clamp the cord until after it has stopped pulsing.

We often hear parents that are very concerned when they see the baby's cord floating near the face during the ultrasound. Doularena made a good point that I remind parents of. That the baby is not dependent on it's airway for oxygen exchange as soon as the head and body emerges. It's still using the cord blood to provide oxygen enriched blood to the baby, allowing plenty of time to free a cord that may be wrapped around the baby.

Baby often goes through a "transition period" after it is born, some take longer than others. A few even thirty seconds can seem like a long time. There are lots of physiological things happening with the lungs so baby breaths air. Talk to your baby, let him/her hear your voice, and gently rub up the spine. Know that as long as the cord is still attached, baby is still getting oxygenated blood from the placenta.
